The haemodynamic changes of 30 hypertensive patients treated with rhomotoxin were studied with mechanocardiogram. Both PEPI and PEP/LVET were apparently increased in our patients, indicating that the myocardial compliance of the hypertensive patients was reduced and their cardiac function was abnormal to a certain extent. Rhomotoxin reduced the cardiac afterload by lowering the blood pressure (from 192±26/114±14 to 143±25/88±11 mmHg) and slowing the heart rate (from 76±13 to 58±10 beats/min) significantly. Its effects on cardiac phases were mainly to shorten QA2I, LVETI and PEPI and to prolong SFW, while other parameters such as PEP/LVET, A/EO, EF and Vcf were unchanged. Thus, as the blood pressure of the hypertensive patients was lowered to a significant therapeutic level by rhomotoxin, the left ventricular ejection would be enhanced without depressing myocardial contraction and its pump function. Furthermore, the myocardial oxygen consumption was markedly reduced (P < 0.001), thus rhomotoxin would help to improve the cardiac function in hypertensive patients.

In 19 anesthetized open-chest dogs the effects of tetrandrine on experimental myocardial infarction were investigated by epicardial mapping and creatine phosphate kinase (CPK) enzyme estimation. In epicardial mapping the ΣST and NST in dogs pretreated with tetrandrine were significantly lower than the control group after 15–30 minutes following ligation of the left anterior descending artery (LAD). The difference of ΣST, NST in both groups reached peak value at 2 hours. If taking the ΣST and NST 2 minutes after ligation as 100%), ΣST in the treated dogs 2 hours after ligation would be 43% vs 91% in the control group, NST in the treated dogs 53% vs 90% in the control group. 24 hours after ligation of LAD the CPK of preserved myocardium of infarcted area in the treated dogs was more than in the control group. The CPK released to blood was obviously less in treated group. These suggested that tetrandrine had some protective effects on experimental myocardial infarction. But because the number of dogs studied was small and the effect on clinical myocardial infarction might be different from experimental model, these observations warrant further investigation.

The carotid pressor reflex can be blocked by rhomotoxin. The intensity of the blocking effect depends upon the original level of blood pressure and is in correlation with the dosage of rhomotoxin given intravenously. Thus the antihypertensive effect may be ascribed to some extent to the blockade of carotid pressor reflex caused by rhomotoxin and is irrelevant to the changes of blood volume, since this depression of reflex is more prominent than that at the same blood pressure level induced by bloodletting.

From February 1980 to October 1981, we have performed double cup arthroplasty of the hip in 10 cases successfully. Here is a report of our experience in clinical application of the double cup we designed and fabricated. Test of wear and tear of several different kinds of material of artificial joint was made. We observed 100 specimens of human dry femur and 80 specimens of acetabulum and reviewed femoral radiographs of 20 normal adults. Important data were obtained on measuring the length and width of the femur and the acetabulum.

In this study we attempt to define the result of pancreas transplantation with the duct left open to drain freely into the peritoneal cavity. Twenty dogs underwent total pancreatectomy, splenectomy, and segmental pancreas transplantation with this technique. Eighteen of the twenty animals recovered normal blood sugar and serum insulin levels 24 hrs after operation. Up to date, the longest survival time is 5 months with excellent pancreatic function. Pancreatic biopsies obtained from animals 45 days after transplantation showed that the islet cells were normal, the acinar tissue was normal on the whole and interlobular connective tissue showed slight proliferation.

In this report we advocate creation of an arteriovenous fistula between the splenic artery and vein at the tail of the graft in order to markedly reduce the incidences of thrombosis in the graft vessels and hemorrhagic pancreatitis.

Our study concludes that segmental pancreas transplantation with unligated duct is a better approach to transplantation of pancreas. This technique is recommended for clinical application.

We applied an indirect hemagglutination inhibition test to detect platelet-associated immunoglobulin G (PAIgG). This test is relatively easy to perform and has good reproducibility. The PAIgG value detected with this method in 45 normal donors was 0–5 fg per platelet. In our series there were 15 ITP patients, of whom two had been in complete remission and had normal PAIgG values. In the remaining 13 cases, 11 were found to have increased PAIgG values (more than 5 fg per platelet). In cases where corticosteroids or splenectomy were effective, rising of the platelet count was associated with PAIgG waning.

Preliminary analysis of clinical data of ITP patients suggested that the clinically diagnosed ITP might be of heterogeneous pathogenesis. A further study is warranted.

Small acoustic neuromas, especially when located within the internal auditory canal (IAC), are difficult to be detected by ordinary X-ray methods. We prefer using small dose Myodil IAC cisternography, with which small acoustic neuromas can be satisfactorily visualized. 27 cases have been examined with this method, of which 13 cases were diagnosed as acoustic neuromas roentgenologically, and 7 of 13 were proved by surgery, 3 of which were small acoustic neuromas and the smallest one had a diameter of only l cm. Beside these, there were 2 cases of cerebellopontine angle meningioma and l case of M ventricle ependymoma.

For examination we adopt lateral recumbent position with the suspected tumor side downward, and X-ray film is taken in A-P projection. The manipulation is simple and the pathological changes can be shown satisfactorily.

    1.
