Precision Vascular Control Before Tumor Resection
Safe and effective hemostasis remains one of the greatest challenges in endoscopic sinonasal and skull base surgery. This issue's cover illustrates a stepwise strategy for precision vascular control before tumor resection. Based on detailed anatomical and endoscopic investigations, the authors established three individualized approaches—transnasal, transmaxillary, and transbuccal—for safe localization and occlusion of the maxillary artery. This anatomy-based strategy enables early devascularization of the tumor, providing a clearer operative field, facilitating meticulous dissection, and reducing intraoperative blood loss. Clinical application in 20 patients demonstrated favorable surgical outcomes, with limited hemoglobin reduction and a low perioperative transfusion rate. This work highlights that anatomy-driven innovation can improve surgical safety and efficiency, offering a practical advance in the endoscopic management of sinonasal and skull base tumors.
