    Metabolic syndrome characterized by tissue insulin resistance, hyperinsulinemia, impaired glucose tolerance, essential hypertension, dyslipidemia, and abdominal obesity and hyperuricemia in the same patient, not accidentally is causing the great interest of researchers. In clinical lecture there are presented data concerning both different variants of the metabolic syndrome (MS) and its prevalence rate in the population. There are given as well diagnostic criteria for MS as algorithm of examination ofpatients. There is considered the concept of the treatment of metabolic syndrome.

    Pregnancy represents the procoagulant state, giving rise in the high prevalence rate of thrombotic complications. An imbalance between coagulation and anticoagulation systems of blood during pregnancy can lead to the development of pre-eclampsia, intrauterine growth retardation, preterm labor, premature rupture offetal membranes and fetal death. These complications are mostly caused by the impaired generation of thrombin as well as changes in the concentration and activity of tissue factor TFPI (tissue factor pathway inhibitor), TAT III complex and protein Z. Nevertheless, it should be noted that the thrombotic complications during pregnancy depend not on the elevation of the concentrations of specific coagulation factors or lack of the activity of anticoagulant proteins, but on the shift in the whole system toward the procoagulant state. These disorders most of all lead to thromboses ofplacental vessels, causing disorders ofperfusion of the affected area. Placental pathology can develop as from the maternal as from the fetal side. Despite the fact that changes in the blood coagulation system occur from the very beginning of pregnancy, and often are adaptive in nature, i.e., do not require special medical intervention, the prompt diagnosis and treatment of hemostatic disorders as main pathogenetic link in the development of thrombosis is extremely important.

    The article presented data of the complex examination of 79 women aged of 24-43 years with uterine form of infertility, including the determination of immunohistochemical markers (ER, PR, Ki-67, CD34, CD56, CD68, CD138, VEGF) in endometrium. As a result of the study there was revealed the decline in the number of CD56-positive cells, decrease or no expression of CD138, the increase in expression of CD34, decreased VEGF expression. The deterioration of implantation processes in patients with uterine form of infertility, presumably is related to the low content of immune competent cells and inhibition of angiogenesis, as well as a weak proliferative activity of the endometrium, as evidenced by a decrease expression of Ki-67.

    It studied 300 outpatients patients urological and gynecological departments of “M.A. Tver'e Medical unit number 9” for 2013-2015 years. We investigated: the incidence of microbial associations in various age groups; the number of reported cases ofAIDS-related on the pathogen; frequency of genital infections among the unemployed; identification ofpatients with factor promiscuity. It was found that among the topical genital infections are currently defined chlamydia, ureaplasma and mycoplasma, viruses of herpes group, which may be associated with HIV.

    Multiple sclerosis (MS) - quite a widespread chronic multifactorial autoimmune neurodegenerative disease of central nervous system, which is based on the disintegration of the myelin sheath (dymyelination). The disease occurs in waves with exacerbations and remissions, often affects women, debuting at the age younger than 18 and older than 45 years. Until the 90s it was thought that pregnancy and the MS are not compatible because of the possible adverse effects ofpregnancy on the course of MS. Over the last 20 years, there was a global inversion of views on this issue.

    The main goal of a prospective study of 84 pregnant women and children who were born at 22-36 weeks gestational age was estimation predictively adverse criteria of cerebral pathology in premature babies. For the evaluation offetal and newborn’s state we performed cerebral vessels ultrasound dopplerography, neurosonography and determine the level of CNS injury biomarkers in cord blood. The study findings suggest the interaction between perinatal outcomes and initial circulation failure of the fetal CNS blood flow in 97% of cases. The concentration of CNS injury biomarkers (S-100 protein, neuro-specific enolase, antibodies to nerve growth factor, myelin basic protein) should be considered as prognostic criteria to determine a degree ofpreterm babies’ CNS affliction.

    Results of treatment of 107 patients who had sacrocolpopexy for the treatment of apical prolapsed were evaluated. Regarding short- and long-term results low postoperative pain score, few complications, satisfactory and good anatomical and functional outcomes in the most cases were noted. Laparoscopic and robot-assisted approach can be considered as a minimally invasive method, the “gold standard” in the treatment of apical and posterior prolapse, however, there is a risk of recurrence of cystocele in the presence of multiple defects pubocervical fascia.

    In this article presented results of microwave endometrial ablation using «Microsulis» technology in 18 patients with endometrial hyperplasia. Somatic pathology, anamnesis and clinical findings, postoperative results were analized. Microwave endometrial ablation is an alternative method of treatment of endometrial hyperplastic processes in view of the minimal invasiveness.

    This article presents our results of laparoscopic hysterectomy among 100 morbid obese patients (BMI > 35 kg/m2). Incidence of somatic disorders, operative time and anesthesia, complications rate, accuracy of final histological diagnosis were analyzed. We believe that laparoscopic approach for hysterectomy is preferable in experienced hands and in well equipped OR due to low complication rate, less traumatism and fast recovery.

    The results ofprevention of HIV vertical transmission were evaluated in HIV-infected pregnant women. The study involved 25 women, all carried out according to the protocol on the prevention of 12.03.2004 № 46/1-3 of Health Department of Ryazan. The results of our research have shown that the prevention of vertical transmission scheme is effective in 100% of all cases, and allows to get healthy children from HIV-infected mothers.

    To establish predictively adverse criteria of cerebral pathology in prematurely born children in comparison with the antenatal indicators offetal condition and outcomes of physiological pregnancies and at the menacing premature birth was the main objective a research. Prospective research ofantenatal ultrasonic cerebral arguments, dopplerometric arguments of circulation of blood in fetus and postnatal indicators in main (n = 40) and control groups (n = 30) is conducted. Statistical analysis of fetus and newborns examination the following adverse indicators of cerebral pathology at prematurely born children are observed: decrease in an index of resistance of internal carotid arteries, decrease in an index of resistance (RI) and the pulsation index (PI) in an middle cerebral artery, decrease in a circulation of blood in Galen’s vein, increase ofRI and PI in vertebral and the bazilar arteries, increase of SDR in uterine arteries and umbilical arteries. Considering existence of high neuroplasticity of a fetal brain and ability to compensate the disturbances arising in the perinatal period, the obtained data can be used as early diagnostics of the beginning injury of central nervous system at premature birth and their treatment at the earliest stages.

    We used the mesenchymal stem cells of the bone marrow of laboratory rats, which are then cultured in vitro and plated onto polypropylene synthetic biodegradable matrices are used for surgical treatment ofpelvic organ prolapse (PROLENE MESH PMM3). After 14 days, the monolayer formed of bone marrow mesenchymal stem cells, fill the cell reticulated matrix. Next is the implantation of the resulting cell engineering design under the fascia in the abdominal wall and tracked the result by histology after 1.5 and 4.5 months. Bone marrow mesenchymal stem cells can be used to improve the biocompatibility of implanted mesh for the surgical treatment of pelvic organ prolapse.
