Paraneoplastic diseases of the skin are classifi ed into obligate, probable, and facultative. Leveres bullouspemphigoid (BP) is referred to facultative ones. Analysis of 67 cases with BP of paraneoplastic genesis (bypublications of U.S. National Library of Medicine) detected gastrointestinal cancer in 18 (27%), breastcancer in 12 (17.9%), and tumors of different location in the rest patients. The authors present a review ofrelevant literature (90 Russian and foreign publications) and 3 clinical observations of BP in the presenceof sigmoid and gastric adenocarcinomas and prostatic cancer.
A pilot study of crystallization patterns of biological substrates (blood serum, saliva, vesical liquid) in12 patients with Duhrings herpetiform dermatitis is presented. Methods of classical crystalloscopy andcomparative tesigraphy were used. The crystalloscopic and tesigraphic facies were evaluated using a systemof parameters, offered by the authors, with subsequent statistical data processing. The results indicated thatDuhrings herpetiform dermatitis was characterized by structurization of biological media, consisting inspecifi c transformation of crystallogenesis towards more active structurization and progress of destructivephenomena in facies. Hence, the biocrystallomic technologies can be used for differential diagnosis inpatients with bullous dermatoses.
A female patient aged 64 years with Rowells syndrome is described. The patient suffered from systemic lupuserythematosus and erythema exudativum multiforme. The clinical picture of dermatosis was liable to varythroughout a rather short period. Antiherpetic acyclovir therapy proved to be effective.
Skin manifestations of syringomyelitis and the development of this disease in a female patient are described.Because of bullous involvement of the shin, the disease had to be differentiated from Leveres bullouspemphigoid.
A single session of photodynamic therapy with intrafocal injection of a Russian photosensitizer radachlorineled to a clinical and cosmetic effect in 6 (66.6%) of 9 patients with atypical solitary keratoacanthomas. This result was signifi cantly higher in comparison with the incidence of spontaneous regression of atypicalkeratoacanthomas.
Spectrophotometric intradermal analysis (SIA-scopy) is a method for life-time diagnosis of skin tumors. It isbased on registration of melanin and collagen levels and microcirculation in the studied tumor. The methodis intended primarily for optimal early diagnosis of skin melanoma. SIA-scopy was used for analysis oftumors of nonmelanocytic origin, which had to be differentiated from skin melanoma.
The results of therapy of patients with surface and T1N0M0 micronodular basaloma by laser-inducedthermotherapy (LITT) and photodynamic therapy with Photolon local photosensitizer are compared. Bothmethods exhibited similarly high effi ciency. An advantage of LITT is its signifi cantly better tolerance andmuch better cosmetic results.
Immunocorrective therapy with immunotropic agents (polyoxidonium or immunofan) and herbalimmunostimulants (collections including buckthorn bark, dandelion root, parsley fruit, fennel, andpeppermint leaves) prevented relapses of multiple skin papillomas in patients with concomitant superinvasiveopisthorchiasis: in 90 ± 9.5% after polyoxidonium and in 90.9 ± 8.6% after immunofan. Patients treatedwithout immunostimulants developed relapses in 72.2 ± 10.6% cases, while a 12-month relapse-free periodensued in just 27.8 ± 10.6% patients.
Published data on a rather rare genodermatosis - Bloch-Sulzbergers incontinentia pigmenti syndrome -are reviewed and a clinical case is described, observed in three generations.
The incidence of genital involvement in focal scleroderma is analyzed in 230 patients with various formsof the disease. A case with genital involvement in a male patient with scleroatrophic lichen associated withBorrelia infection is described. A total of 19 publications on the genital location of focal scleroderma arediscussed.
Published data on the contribution of microorganisms acting as superantigens in atopic dermatitis arereviewed. The functions of Toll-like receptors (TLR) in the skin and the role of TLR2 in stimulation ofcongenital immunity are discussed. The author shares her experience gained in the use of these drugsin topical therapy for atopic dermatitis: gamma Cu-Zn+ with TLR2-Regul innovation complex (Uriage,France), stimulating the early reactions of congenital immunity in complicated course of dermatitis.
A rare sarcoidosis is described: Besnier-Tennesson lupus pernio. The clinical picture was similar to that oflepra: similar changes in the skin integument and pain sensitivity disorders were observed.
Catelicidine (antibacterial peptide) in the LL-37 form is characterized by anti-infl ammatory, immunoregulatory, and bactericidal effects towards P. acnes. Serum levels of this peptide were measured in30 patients with acne of different severity (I, II, and III degree) by EIA before and after highly intense bluelight (405-420 nm) phototherapy. The level of LL-37 peptide directly correlated with the disease severityand normalized after phototherapy.
Clinical effi ciency and tolerance of external Sensibio therapy is evaluated in 28 patients aged 18-46 yearswith rosacea and seborrheic dermatitis.
The morphology and functions of normal skin of different anatomical appurtenance from representatives ofdifferent phototypes have been evaluated. The study was carried out in 30 volunteers aged 20-30 years with skinphototypes I, II and V. VI. The structural parameters of the skin were studied by the optic coherence tomography(OCT) using an optic coherent computer-aided tomography visualizer (Russia) for noninvasive studies of theinner structure of human surface tissues with longitudinal and transverse resolution of 20. The functionalcharacteristics of the skin were evaluated by the Milti Skin Test Center MC 750 (http://nicosmetics.ru). Theresults were statistically processed by Shapiro-Wilk and Mann-Whitneys tests. The data are presented by medianswith the fi rst and third quartiles. Morphological characteristics of normal skin in representatives of differentphototypes, associated with certain functional parameters, have been detected. The detected morphofunctionaldifferences were more manifest on the skin sites maximally exposed to ultraviolet irradiation.
