The prospects of development of the system of medico-social rehabilitation of invalids and increase of role of medico-social expertise has been examined in this article.
The article is devoted to actuality and necessity of the integration of medical and social care. The main structures of this care are underlined here. The experience of some new structures has been characterized. There are the proposals for some measures which are necessary for complex rehabilitation of patients with special needs.
Success of rehabilitation depends largely on the structural and organizational forms and working methods of the service, continuity of connection in stationary, outpatient and sanatorium stage of rehabilitation. At the same time, the process of rehabilitation of children with disabilities should be implemented primarily through the volitional capacity of the parents, since the identity of the rehabilitated child has not yet been formed.
Problem of determination of group of disability on the basis of objective tests of the clinical and functional state of organism with violation of ear is not decided yet. The normative documents for determination of group of disability, classification of degree of hearing loss used in practice of medico-social expertise and international classification of degree of hearing loss used in Health Care System of Russian Federation have been presented in this article.
In the article the results of the study of legal principles of the native system of technical rehabilitation of disabled persons have been described. A comparative analysis of Russian and international applicable legislation for regulation of legal relations in this sphere has been given. Proposal to change the actual state of things targeted at improvement of quality of life of disabled persons have been proved.
In the article there have been studied the analysis of dynamics of primary disability of adult population of the Nizhegorodskaya region for 18 years. The incurrence of supervision units is 707309 people, examined at the establishments of medical and social expertise for period 1992-2009. There has been defined a structure of primary disability (674137 persons during all investigated period), taking into account the groups of disability, age, sex, place of residence. The grade locations of classes of diseases have been determined in the structure of primary disability.
A structure, level of primary disability of adult population and requirement in rehabilitation measures have been studied for 2007-2009 years. Intensive index of primary disability, incurrence first acknowledged invalids among adult population had a tendency to the increase during all probed period. In a contingent first acknowledged invalids for probed period the invalids because of illnesses of blood circulation system, musculoskeletal system, nervous and endocrine systems, malignant new formations, invalids are II group and at least female have been prevailed. Requirements in development of individual program of rehabilitation for all probed period was made 100%.

Timeliness of using of bandage as one of principles of conservative orthopedic treatment (especially on the early stages of joint lesions) is the guarantee of efficiency of rehabilitation. Application of bandage helps to redistribute loading on joints, to compel the different groups of muscles more fully to join in work and as a result to prevent progress of diseases on the early stages, to improve quality of life of patients, help them to go back to normal life without limitation former loadings.

This research was demonstrated the necessity of creation of the specialized rehabilitation services of social-psychological rehabilitation.
The modern state of problem of rehabilitation of invalids due to malignant formations of the breast with postmastectomy syndrome has been presented: indexes of morbidity, disability, rehabilitation from data of literature. Drawn conclusion that the individual program of rehabilitation of this category of invalids can plug in itself except the classic methods of medical rehabilitation but also modern methods.
There has been presented the review of scientific literature for the last 20 years. The features of psychological condition of patients with the consequences of spinal cord injury, having an important value for forming of strategy of psychological rehabilitation have been described in this article.
