May 14, 1999 marks 185 years since the opening of the Medical Faculty of the Imperial Kazan University, a significant event in the history of higher medical education in our country. After the medical faculty of Moscow University (opened in 1758) and the St. Petersburg Medical and Surgical Academy (1798), this faculty became the third forge of domestic medical personnel in the 19th century in Russia.
The ultraviolet laser spectrofluorimetry method for the diagnosis of urgent surgical disrases of abdominal cavity organs and the determination of their vitality is determined. The experimental studies showed the possibility of the use of Nd: YAG-Iaser for sutureless connection of uterus wound edges and geometric recovery of serous integument on the basis of tissue “welding”. The use of the laser “welding” suture in the clinic made it possible to decrease by a factor of three the frequency of occurence of pyo-inflammatory complications after caesarean section and uterus supravaginal amputation
The comparative estimation of results of cesarean section in 269 puerperas depending on suture material and peritonization method is given. Catgut and resolved suture material vicril (polyglactin 910) werw used for trachelorrhaphy. Peritonization was performed traditionally and using laser “welding” surure of the peritoneum. The minimum number of complications is revealed in the group where uterus integrity was recovered using vicril suture in combination with laser photocoagulation of the peritoneum. The largest number of complications was observed in using catgut.
The results of experimental studies made it possible to draw a conclusion on the reality of cerebral vessels reactivity recovery using drugs with primary neurometabolic activity, in particular, dimephosphone, sermion and pyracetam. The advantages of low-toxic nonauticholinesterasic organic phosphorus compounds among the correctors of regulatory mechanisms of circular provision of the brain are shown.
The increase of systemic and regional arterial pressure, the disorder of microcirculation in bulbar conjunctiva and eye retina take place in the combined craniocerebral injury. The effective method in complex pathogenetic treatment of this type of the injury is novocainic trigeminovagosympathetic block including the effects of the Zhakov trigeminosympathetic block and the Vishnevsky vagosympathetic block. The complex pathogenetic treatment method of the combined craniocerebral injury using trigeminovagosympathetic block abates pain, decreases arterial pressure, normalizes microcirculation and finally strengthens reparative and proliferative processes in damaged organs and tissues.
The results of clinicopsychopathologic and pathopsychologic examination of 70 patients of various age groups with neurotic disorders are given. Some age peculiarities are noted: a predominance of pathogenic monovariant and polyvariant types of prediction in all age groups with their dominance in children and elderly persons, a predominance of various antici patedly mediated emotional phenomena in patients of various age groups: perplexity affect in children, disappointment affect in elderly persons, offence affect in teenagers and adults.
In examination of 240 women with alcoholism the spontaneous in initiation and autochthonous in course alcoholism variant which does not depend on psychotraumatizing factors and is formed in early young age, was revealed in 70 (29,2%) of the patients. Numerous predisposing factors, hereditary and genetic, microsocial, premorbid and personal, constitutional and psychologic factors were revealed. These patients respond with difficulty to phychotherapeutic treatment, so their treatment is usually limited by the use of medicines.
Augmentin in the form of aerosol is used as an independent remedy of antibacterial therapy in a dose of 100 mg dissolved in 2 ml of water for injection 2 times per day into the nasal sinus or maxillary sinuses by drainage using a sprayer. The nasal sinus and sinuses were washed properly by a physiological solution. The drug was used in 22 patients with acute sinusitis. Acute maxillary sinusitis was diagnized in 16 patients, acute ethmoiditis — in 2 patients, acute frontal sinusitis metopantritis — in 4 patients. The positive dynamics was noted in all patients after the begining of the treatment.
As many as 260 patients with voice disorders caused by pathology of the supercervical part of vertebral column are examined. This disease in literature is determined as functional dysphony and is characterized by the absence of visible pathology in the larynx. However it is shown that the larynx and surrounding organs can be involved into the myofixation zone and give the adequate response to the pathology of the cervical part of vertebral column.
The manifestations of muscular syndrome affecting the gravity and prediction of the disease take place in the most widespread and invalidizing rheumatic diseases: osteoarthrosis, rheumatoid arthritis and ankylosing spondylarthritis. Paraclinical studies showed heterogeneity of pathogenetic mechanisms in lesion of skeletal muscles in rheumatic diseases. The differentiated methods of the treatment of patients with osteoarthrosis, rheumatoid arthritis and ankylosing spondylarthritis taking into account the clinical picture and pathogenesis of muscular syndrome, as well as the diagnosis criteria and classification of muscular system lesion are suggested.
For the purpose of increasing informativity of X- ray methods in using them in the diagnosis of large intestine diseases including the differential diagnosis of nonspecific ulcerative colitis and Crohn disease 27 patients with the given diseases were examined using a modified method-computer tomography with intraenteric contrasting. The computer-tomographic sighus characteristic for colitis and Crohn dicease are established.
Dislocations and fracture-dislocations of the wrist bones are severe and fairly frequent (up to 35% of all cases of skeletal fractures) injuries of the wrist joint. With chronic dislocations and fracture-dislocations of the wrist bones, treatment presents significant difficulties.
Herpetic infections are the most common cause of chronic human viral diseases. However, until recently, these infectious diseases were not regarded by doctors as a rather serious pathology, and the attention of domestic clinicians and virologists has attracted only in the last 15-20 years.
The principal directions of the formed project of the Republican program having a special purpose “Healthy heart” in 1999 — 2009 — the program of primary and secondary prevention of the basic cardiovascular diseases in Tatarstan Republic are given. Recommendation of suggestions of this program will make it possible to economize up to 40% of means by decreasing the number of patients as a whole, by reducing expenditures connected with payment of disability pensions and other medicosocial expenses.
The organizational structure of uronephrologic service based on the 35-year experience is suggested. The basic principles of the service functioning and the necessity of obligatory reglamentation of the volume and type of medical and diagnostic procedures among children’s medical and prophylactic institutions are substantiated. These measures increase the level of active revealing patients, decrease the number of diagnostic and medical errore, increase the efficiency of work of specialized subdivisions of the service, bring the specialized medical service to children with uronephrologic pathology nearer to rural and remote from the centre industrial regions.
According to E.I. Pogorelova et al. [1], only general quantitative indicators, which are clearly insufficient to determine the need for additional training of doctors of this service and its planning.
The regulation of the fluid state of the blood depends on many factors. One of the most important among them is plasmin activity. Under normal conditions, because of the pronounced anti-plasmin properties of blood, it is weakly manifested. Currently, it is often determined by lysis of a clot of the euglobulin fraction of plasma.
The sharp politicization of the history of science in the USSR erased from its pages the names of scientists who were forced to leave Russia after the October Revolution.
