    To study the association with diabetes mellitus type 2 we performed analysis of the distribution of frequencies of alleles and genotypes two polymorphic markers of TCF7L2 gene. The study included groups of T2DM patients and unrelated controls of Russian origin. Analysis of the distribution of frequencies of alleles and genotypes of the polymorphic marker rs7903146 of TCF7L2 gene showed the presence of association with T2DM in Russian population, while for the marker rs12255372 was not found statistically significant associations with type 2 diabetes. We can conclude that in Russian population TCF7L2 gene is associated with development of T2DM.

    The impact of changes dialysis program on ENMG rates in patients with a terminal stage of chronic renal insufficiency receiving regular dialysis treatment was studied. The study made it possible to determine that the strengthening of the dialysis program with the increase in KT/V to 1.4 promotes regression of neurological manifestations of uremic polyneuropathy and improvement of neurophysiological indicses of functioning of the peripheral nerves in patients with a terminal stage of chronic renal insufficiency. Significantly improved indicators of functional activity as axial cylinders of the peripheral nerves and the myelin sheath. Optimisation of dialysis program allows to improve the functional activity of peripheral nerves already by the end of the first month, the maximum therapeutic effect at 6month treatment and to obtain the steadfast compensation sensory and motor deficit of uremic polyneuropathy. Thus, for the correction of uremic polyneuropathy it is expedient to strengthen the dialysis program with the increase in KT/V to 1.4.

    Description of case reports of a patient with diagnosis of dilated cardiomyopathy granting of a historical reference and modern ideas about the etiology, pathogenesis, course, and management of the disease. Performed surgical treatment (orthotopic transplantation of putrid heart), along with medication, allowed to improve the quality of life and prognosis of the disease.

    The case report of the patient with complaints of recurrent sudden breathlessness at night is presented. The approach of the diagnosis and therapy is introduced. The effectiveness of the treatment of CheyneStokes respiration by adaptive servoventilation has been shown.

    Diaphragmatic injuries are quite uncommon and often result from either blunt or penetrating trauma. Acute traumatic rupture of diaphragm may be unrecognized and there is often a delay between the injury and the diagnosis. This article reviews the case reports of three patients. The first patient had a penetrating trauma of left diaphragm. The diagnosis was made 5 days later. The second patient had herniation with kidney, colon and intestine through the left diaphragmatic rupture. The third patient had right diaphragmatic rupture and herniation with hepar, сolon and stomach. Both cases were presented as early undiagnosed injuries.

    The article describes a rare clinical case of primary amyloidosis with heart lesion and severe hydropericardium. The patient was early considered as manifestation of ischemic chronic heart failure. The diagnostic difficulties of this case are described.

    In the article the authors present analysis of current conventional methodological and technological approaches to contemporary therapy of cancer and certain malignant neoplasms. Based upon the revision of available concepts of clinical oncology, the scientific and theoretical validation of novel approaches to the development of innovative technologies of proteomebased regulatory tumor therapy was attempted. The authors defined their original viewpoint on the development of the preparations for personalized therapy of cancer and proposed a new methodology and alternative approach to the malignant tumors therapy when a cancer stem cell becomes the main target of anticancer therapy. The authors showed that proteome of a cancer stem cell is the most stable information structure of a cell and can be perceived as the main therapeutic target.
