    Purpose. To assess the efficacy and safety of implanting a supplementary toric intraocular lens (IOL) in the ciliary sulcus in one eye and a toric IOL in the fellow eye in highly hyperopic patients with concomitant astigmatism. Methods. This study included highly hyperopic patients (axial length 21.3-22.0 mm) with concomitant regular corneal astigmatism. The group consist of 6 patients aged from 40 to 76 years. Supplementary IOL (Sulcoflex Toric 653T, Rayner) was implanted in the ciliary sulcus in the pseudophakic eye 1 month after previous phacoemulsification surgery. A toric IOL (AcrySof IQ Toric, Alcon) was implanted in the fellow eye. Postoperative follow-up visits were performed at 1 week, 1 month and 6 months. Results. Postoperatively, in all patients UDVA (uncorrected distance visual acuity) improved and remained stable throughout the follow-up period. Lower visual acuity in the eyes with toric IOLs is associated with errors in IOL calculation, occurring often in “short eyes”. Conclusion. Using different IOL types for astigmatism correction in highly hyperopic patients is justified and can give good visual results. A “short eye” is not a contraindication for supplementary IOL implantation, but it is necessary to perform laser iridotomy to minimize the risk of pupillary block.

    A retrospective study of 3346 clinical records of patients with the diagnosis “Uveitis” (ICD-10 codes - H 20, H 22, H 30) hospitalized in the “Clinical ophthalmologic hospital named after V. P. Vykhodtsev” in Omsk from 2005 to 2012 was performed. The prevalence of uveitis decreased by a factor of 3.2 in comparison with the prevalence measured from 1995-2004 data. We also found a statistically significant decrease in the incidence of the recurrent forms of uveitis Compared to the time frame of 1995-2004, the immunoblot (West-ernblot) test was use more frequently. A comparative analysis of laboratory diagnostic methods was carried out. An increase in application frequency of a method for etiology verification - immunoblot (Westernblot) - is noted. A comparison between the prevalence of iridocyclitis and that of chorioretinitis in various age groups was studied. The results showedthat iridocyclitis is more common in young working age population.

    Objective. To study the relationship of post-operative ocular hypertension with the encapsulation of Ahmed valve’s plate. Methods. The standard Ahmed valve implantation was performed in 238 patients aged 18-86 with multiple glaucoma surgeries (39 %), secondary neovascular (36 %), pseudophakic (23 %), juvenile (8 %), uveal (5 %) and post-traumatic (2 %) glaucomas. A routine ophthalmic examination was performed in 1 week, 1, 3, 6, 9, 12 and 36 months after surgery. Results. Excision of the fibrous capsule as the only way to cope with post Ahmed valve ocular hypertension was performed on 16 patients 3-36 months after surgery. Macro- and microscopic analysis of the excised capsules was done. In all cases, preparations of 1.2-2.2 mm thickness were obtained. We found a bilaminar structure of the capsule, the inner surface consisting of densely-packed collagen fibers, whereas the outer layer is represented by a loose fi-brovascular layer. Suspension of encapsulation is possible at initial stages of scarring, by applying ocular massage, needling, revision of the surgical area, and injection of antimetabolites.

    Purpose. To study the effect of vitrectomy in retinal detachment (RD) treatment on the quality of life (QOL) of patients. Methods. We examined 67 patients who underwent surgical treatment of RD. QOL was assessed by VFQ-25 questionnaire before surgery and after 1 week and 6 months of it. Results. When assessing QOL before surgery, there was a significant reduction of the total QOL index by an average of 35% in comparison to the control group (p < 0.001). In the late postoperative period, a progressive increase of the total QOL index and visual function was recorded. Conclusion. Vitrectomy for the treatment of retinal detachment improves patients’ visual function and quality of life.

    There are different methods of IOL fixation in case of inadequate capsular support, one of them being IOL implantation into the ciliary body sulcus. Foldable aspheric Akreos IOLs produced by Bauch&Lomb were implanted in 212 patients, who had had a dystrophic or traumatic crystalline lens subluxation of 3rd degree, luxation into the anterior chamber, post-traumatic aphakia and in those in whom a posterior capsule rupture occurred as a phacoemulsification complication. A method of IOL suturing in the ciliary body sulcus in the absence of capsular support for the foldable IOL implanted through a small incision was worked out. A visual acuity analysis revealed high functional results, in particular in cases of small incision implantations. Among postoperative complications, hemorrhagic ones were observed most often. It was shown that IOL Akre-os AO implantation into the ciliary body sulcus could be used in case of inadequate capsular support.

    The development of the dry eye syndrome in glaucoma patients is a pressing challenge during last 10 years. As reported by multiple investigators, the main causes for the development of corneal and conjunctival xerosis in such patients are: toxic action of preservatives in hypotensive ophthalmic medications, pharmacological effect of beta-blockers, and corneal trauma at the time of diagnostic manipulations. A meaningful action to prevent this disease is a to switch to either a preservative-free hypotensive ophthalmic medications, or to those containing a non-toxic preservative. A review of the literature was also done to find the causes, prevention and treatment methods of the dry eye syndrome in glaucoma patients

    In this review, the collagen cross-linking is considered as one of promising methods to treat corneal diseases. Present-day ideas about the cross-linking’s mechanism of action and indications for this procedure are covered. Results of this method application for various nosological forms of ophthalmic diseases are presented both from literature data and own experience. Main directions of this method’s further improvement are designated.

    The optical coherence tomography is a modern method to assist the ophthalmologist examine the eye fundus. Tomographs have a very high resolution and give the ophthalmologist a in real-time mode in vivo a detailed examination of retinal, optic nerve and choroidal structures. A continual improvement of this technique offers great opportunities and is not only of scientific but also of practical interest.

    In this article, an analysis of medical records of 51 patient who underwent eye preserving treatment of choroidal melanoma (brachytherapy or combination of methods) with subsequent secondary enucleation is shown. According to their initial sizes, tumors were divided into small, medium and large (11, 13 and 27 eyes, respectively). In small melanoma cases, the period of time before secondary enucleation was the longest, in cases of medium size tumors, it was more than 1.5 times shorter, and in large choroidal melanomas - two times shorter (р = 0,02). A characteristic feature of the small and medium-size melanoma groups was a low rate of postradiation complications (2 of 11 eyes, and 2 of 13 eyes, respectively). In large melanomas, morphologically the tumor was proven in 23 out of 27 eyes, and postradiation complications as an indication for enucleation were much more frequent (13 eyes).

    Atopic keratoconjunctivitis (AKC) is a nosologic form seen very frequently in multifunctional hospitals, as it co-exists with systemic diseases. In the present article, a clinical case of an extended treatment of a patient without any permanent improvement is described, because AKC was not recognized until cataract development and hospitalization for cataract surgery. The surgical procedure served as a releaser for AKC exacerbation, what allowed to put a correct diagnosis and to work out an adequate treatment regimen, based on immunomodulating Restasis® eye drops.
