The article is devoted to the analysis of the dual attitude in psychiatry to patients with the behavioural pathology including personal, addictive and sexual disorders. On the one hand, these disorders are admitted as the psychiatric diagnoses classified by ICD and DSM. On the other hand, such patients actually are refused in an opportunity to take all lawful advantages of patients (sick lists, disability). Process of “medicalization” of behavioural deviations and insufficient scientific validity of inclusion their psychiatric classifications is analyzed.

The article discusses the modern aspects of the organization of the central nervous system (CNS). First of all, the importance of cerebral connections is emphasized. It is discussed about the concept of the human connectome, the principles of its construction. The paper considers the mechanisms of the ability of forecasting events and creative abilities. It is clarified that in the functioning of the CNS the leading role plays the feature of the brain, related to its spontaneous rhythmic and chaotic activity. Separately is analyzed the resting state activity of the brain. The paper is dedicated to the global principle of the brain functioning- the achievement of the goal (i.e. communication) with minimal energy consumption. The article is concerned with the hypothesis about the discrete nature of the brain activation/functioning in connection with the occurrence of the “mixed-mode oscillations”. The processes of recovery after stroke are analyzed. The conclusion is that the currently received data allow to mark off a separate area in the field of neuroscience - system psychoneurology, which combines clinical, neuroimaging and mathematical data. This integrated approach opens new possibilities for researching the brain working.

The aim of the work was to study the features of the clinical picture of schizophrenia with alcohol abuse comorbidity, taking into account the possible influence of gender and genetic factors. The association of comorbid alcohol abuse with a long duration of an exacerbation in women has been revealed. The connection of the Val158Met allele of the COMT gene with an earlier debut and a deterioration in remission in men has been found. The development of alcohol abuse in patients with schizophrenia is associated with an earlier debut of the disease.

The purpose of this study was to make a comparative analysis of anticipatory consistency, tolerance to uncertainty and anxiety among pharmacists and logisticians. Differences in the findings between a group of people whose activities were more or less uncertain or specific were identified, and the relationship between anticipatory solvency, tolerance to uncertainty and anxiety among respondents in these groups was also found. Based on the research data, the authors conclude that individuals who are in a situation of professional activity associated with uncertainty will show better spatial prognostic competence, tolerance to uncertainty and lower anxiety in comparison with individuals in a more defined professional environment who show more anxiety and intolerance to uncertainty.

There was conducted clinical research and analysis of monitoring visual and somatosensory evoked potentials during 5 years of follow-up in 30 patients with clinically isolated syndrome, in 25 of them the onset of the disease was observed with retrobulbar neuritis and 50 authentic with multiple sclerosis. There were found prognostically significant risk factors for the transition of clinically isolated syndrome into a reliable multiple sclerosis: an increase in the latent period of visual evoked potentials over R100 data healthy to 150±1,7 MS, P2 to 146±1,3 MS, N75 to 115±1,8 MS and a reduction in amplitude and increase in latency somatosensory peaks in multiple sclerosis during the year of observation, established their diagnostic value and high information to identify processes of demyelination in the brain and spinal cord.

The results suggest that there is a significant relationship between clinical characteristics and chronotype categories in patients with depression (n=100). Eveningness was associated with a strong family history, earlier onset. These data indicate an important role of psychometric testing (MEQ-SA) based on a patient’s individual characteristics.

Authors conducted a multi-stage study to improve the early diagnosis of cerebral metastases of peripheral non-small cell lung cancer (PNSCLC). Based on the data of retrospective analysis and clinical material, the frequency of metastatic brain disease in patients with PNSCLC was analyzed. The dependence of the frequency of cerebral metastases on size, histological structure of the primary tumor, and the involvement of regional lymph nodes was studied. A preventive use of MRI in the detection of cerebral metastases of PNSCLC demonstrated significant improvement. The diagnostic capabilities of MRI with intravenous contrast and PET/CT with 11C-methionine in the detection of cerebral metastases are compared.

The aim of the study was to compare the social functioning (SF) indicators in males and females with paranoid schizophrenia. The study included 116 patients with paranoid schizophrenia up to ICD-10 diagnostic criteria. The first group (males) included 62 patients, mean age 45.6 ± 10.2 years, the second group (female) included 54 patients, mean age 46.1 ± 10.7 years. Clinical-psychopathological, clinical-archive and statistical methods were used in the study. It was found that the level of SF was higher in females in the areas of interpersonal relationships, marital (excluding sexual) relationships, sexual relations (including in the family), and in the everyday life organizing. Females often visited shops and used public transport. No differences between groups were obtained in the indicators of disability, present time employment, length of professional occupation, dynamics of financial position during the illness, family status. This may be explained by the age and duration of the disease. Gender mainstreaming can contribute to a more differentiated approach in providing psychosocial care to patients with paranoid schizophrenia.

There was submitted the review of literature about the relationship between vitamin D deficiency and benign paroxysmal positional vertigo. It is showed that vitamin D deficiency is linked with more frequent and intense attacks of vertigo, and classic treatment has only a temporary effect. Replacement of vitamin D deficiency leads to the decrease of relapse rate and improvement of the life quality in patients with this disease.

Some questions about the etiology, pathogenesis, classification, clinical manifestations are revealed. Hypothesis of formation of main movement disorders in CP, especially such as spasticity and dystonia are discussed. Modern techniques of rehabilitation in childhood and adult in CP are described. A lot of foreign trials dedicated to epidemiology, rehabilitation and main clinical problem of CP patients. Small Rho GTPases act through the Rho-associated kinase (ROCK) activated by them, and control the migration, proliferation and death of neural cells and reactive astrogliosis. The Rho/ROCK signal pathway mediates the effect of endogenous axons growth inhibitors from myelin, oligodendrocytes and glial scar and it is activated in spinal cord trauma, which leads to an increased inflammatory response, neuropathic pain, demyelination, cell death, inhibition of axon growth, and impaired function. After spinal cord injury under conditions of pharmacological inhibition of Rho/ROCK was shown increased level of growth and branching, increased plasticity of neuronal structures and intercellular communications, neuroprotective effect, increased remyelinization and improved function. In addition to selective inhibitors of RhoA (ADP-ribosyltransferase C3) and ROCK (Fasudil, Y-27632 and Y-39983), the ability to inhibit Rho/ROCK was indicated for non-steroidal anti-inflammatory drugs such as ibuprofen and indomethacin, as well as for statins.

In the previous issues of the journal psychiatrists’ discussion on questions related to the medicalization of moral choices and nonconventional behavior has started. This article contains answers to Dr. I. Zislin objections to the arguments put forward by me about the antiscientific concept of “personality disorders.” There are additional facts demonstrating the theoretical unprovability of the concept, inadequacy of the diagnostic criteria and the harm caused by the application of the “personality disorder” category in practical psychiatry.
